Telehealth appointment for new and existing patients.   This is for the Hormone Replacement Program Specifically.

•  This is a quarterly or monthly subscription service.
• If you are new to the program you must have a visit every month for the first three months or  until you are told by your provider to transition to a quarterly program
• If you are already on a program  and have been seen on a quarterly basis before, please select a quarterly option.
• This decision is based off of  stability, clinical response and other objective factors
